Candidates must have experience in Hospice or Home Health.  We need RN's for S. Dallas, Arlington, Fort Worth & Garland, TX
 
Currently seeking a compassionate, emotionally mature professional to perform under the direct supervision of the Director of Clinical Services.
 
This position is responsible for administering nursing care for hospice patients of all ages in their place of residence and assumes the responsibility for coordinating care with the interdisciplinary team, patient/family and referring agency
 
Essential Functions:
  • Assess hospice patients to identify physical, psycho-social and environmental needs as evidenced by documentation, clinical records, case conference, team report and evaluations
  • Implement/develop/document the plan of care to ensure quality and continuity of care
  • Supervise and provide clinical directions to the HA/LVN to ensure quality and continuity of service provided
  • Provide care utilizing infection control measures that protect both the patient and staff (OSHA)
  • Monitor assigned cases to ensure compliance with requirements of third party payer
  • Demonstrate commitment, professional growth and competency
  • Promote company philosophy and administrative policies to ensure quality of care
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned
  • Provide effective communication to patient/family, team members and other health care professionals
  • Meet mandatory continuing education requirements of the agency/licensing board
  • Attend IDT meeting, participate in the patient care planning process and collaborates with the IDT to promote coordination of patient care
 
Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
 
Licensure: Current Texas state license as a Registered Nurse; Current Texas Driver’s License
 
Experience: Two years of experience as a Registered Nurse in a clinical care setting (hospice/homecare highly preferred)
 
Skills: Nursing skills as defined by generally accepted standards of practice; Good interpersonal skills
 
Transportation: Reliable transportation; Valid and current auto liability insurance
 
Criminal History: Must agree to and pass criminal history checks
